Child Care Required Forms for Admission

Below are all documents needed to enroll in our child care program. Please note that every form must be completed prior to a child entering the program. Please read over each document and description carefully.


  • All parents and guardians are REQUIRED to provide their child's Forms for Admission prior to their child attending the child care program. Maryland child care regulations require the Child Care Division, as your child care provider, to maintain these required forms for your child while they are in attendance at our program. 
     
  • Please understand that your child(ren) will NOT be permitted to attend without this information on file at the child care center.
     
  • How to Upload Your Forms
    1. Log into your online child care account.
    2. Access the required forms shown below.
    3. Click the upload button for each required form (all files must be saved as a PDF).
    4. The document will move from the "Forms That Need Attention" section to "Approved or Uploaded Forms."
       

Please be advised that SACC/CEC operates as a separate entity from the school. Therefore, it is the parent’s responsibility – NOT the school’s responsibility - to provide the forms listed below.

If your child has never enrolled in any of our child care programs before, please fill out and submit these required forms prior to their first day. 









Medical forms must be renewed on an annual basis or one year after health provider has signed it (as indicated by the expiration dates your health proider has noted on the forms):
