The Senior Care program provides coordinated services, including screening, assessment, case management, and gap-filling services, to eligible individuals through local aging, health and/or social services agencies.

The Senior Care Program assists county residents aged 65+ with moderate to severe disabilities who are at risk of institutionalization. Income and asset verification required. 

Services Provided *Provided on an as-needed basis

  • Case Management
  • Home visits every 30-45 days
  • Medical Alert Devices*
  • Assistance with applying for homeowner’s and renter’s benefits*
  • Transportation Assistance*
  • Medical Adult Day Care*
  • Assistance with personal care* (up to 6 hours per week)
  • Meal assistance*
  • Gap Filling Funds to purchase medical or other safety equipment*
  • Housekeeping*
  • Incontinence supplies*
