This is responsible work involving the determination of client needs, linking to appropriate community resources and follow-up to ensure receipt of needed services. The I & A Specialist provides Options Counseling, advocacy and short-term case management work on behalf of seniors, special needs and disabled citizens, and their family members/caregivers. Work involves collaborative and cooperative work with community agencies, helping clients obtain services/housing/other resources, advocating on behalf of clients and families, screening regarding entitlement programs and assisting with application processes via telephone, walk-in contacts, and home visits as needed. Work may also involve cross-training with the State Health Insurance Program (SHIP) to provide Medicare support during open enrollment periods and as needed throughout the year. Occasional outreach on evenings and weekends may be required. Additional duties may be required.
Minimum Qualifications:
Graduation from an accredited four-year college or university with a Bachelor’s degree in psychology, counseling, social work, human service, or a related area; knowledge of basic counseling/human services principles; knowledge of social insurance programs; two or more years of experience in working with the elderly and/or adults with disabilities in human service systems.
NOTE: Graduation from an accredited four-year college or university may be substituted with one (1) or more additional years of experience in working with adults with disabilities or elderly adults in human services systems, for a total of three (3) or more years of experience.
Preferences:
Preference given to those who have knowledge, skills and abilities navigating service systems with people with disabilities. Case management experience preferred.
