SOAR Services Provided
The SOAR Program provides case management and services to allow older adults in need of long-term services and supports to age in a location of their choosing with appropriate services, supports, and housing opportunities. Services include:
- Case Management
- Home and community-based supports, including personal care, homemaker/chore services, meals, transportation, safety related minor home repairs/modifications, meal services, adult day care, medical supplies, emergency response systems, respite care, and other supportive services
- An assisted living subsidy for monthly housing fees in an approved assisted living facility
These services are supported by state grant funding and are subject to annual funding availability, as well as annual redetermination of individual eligibility.
