About
Community First Choice (CFC):
This program is an option to aid older adults and individuals with disabilities who would otherwise require nursing home or immediate care facility care.
Eligibility guidelines:
- To be eligible an individual must be a Maryland resident and must have community Medical Assistance.
- Individuals must meet a nursing facility level of care.
Services available include:
- Personal Assistance
- Nurse Monitoring
- Personal Emergency Response Systems (PERS)
- Supports Planning
- Home Delivered Meals
Community Personal Assistance Services (CPAS):
This program provides personal assistance services, including assistance with activities of daily living, to Medicaid participants.
Services include:
- Personal Assistance
- Supports Planning
- Nurse Monitoring
Eligibility guidelines:
- To be eligible an individual must be a Maryland resident and must have community Medical Assistance.
- CPAS does not require the individual to meet a nursing facility level of care to participate. The individual must get a CPAS level of care.
These programs have no caps on the number of eligible individuals who can be enrolled. This means that anyone who meets the eligibility requirements is able to receive services through these programs.
