Supporting Older Adults with Resources (SOAR)

Effective July 1, 2026, Maryland’s Senior Care, Senior Assisted Living Subsidy (SALS), and Congregate Housing programs have been consolidated into Supporting Older Adults with Resources (SOAR). Funded by a grant from the Maryland Department of Aging, this community-based program provides case management and gap-filling services to help older adults remain living safely in the communities. SOAR Services Provided

SOAR creates equitable access to the following long-term services and supports by standardizing eligibility, utilizing evidence-based tools to assess risk, and providing person-centered resources that maximize independence.

  • Case management support to include integrated screening and evaluation, development of an individual plan of care, coordination of community based services, and resources for caregivers
  • Home and community-based supports that may include personal care, homemaker/chore services, meals, transportation, safety related minor home repairs/modifications, meal services, adult day care, medical supplies, emergency response systems, respite care, and other supportive services
  • An assisted living subsidy for monthly housing fees in an approved assisted living facility

These services are supported by state grant funding and are subject to annual funding availability, as well as annual redetermination of individual eligibility. .
