Overview
Nurses or social workers in the AERS program provide an in-person assessment at the client’s home. The assessment looks at the bio-psychosocial needs of the client. The AERS assessor then discusses with the client or family members, as appropriate, a plan that identifies community resources for which the client may be eligible.
- Assessments by licensed registered nurses and licensed social workers
- Plans of care that address a client’s needs
- Referrals to community resources that are both eligibility-based and available to clients
- Assessment of individuals who have intellectual disabilities or mental illness and require nursing home placement
