Adult Evaluation and Review Services (AERS) serves both individuals with chronic illness and disabilities who require assistance in the community and Medical Assistance funded care.
Nurses or social workers in the AERS program provide an in-person assessment at the client’s home. The assessment looks at the bio-psychosocial needs of the client. The AERS assessor then discusses with the client or family members, as appropriate, a plan that identifies community resources for which the client may be eligible.
