Please be advised, if you are requesting multiple citations for the same event, please complete only ONE request and include the list of honorees and any special wording in the space provided below OR include the list as a Word or Excel attachment.

*NOTE: For retirements, please include position held, years of service, and company or organization. For Scout related citations, please include Troop number and service project (where applicable). For In-Memoriam requests please include both the Date of Birth & the Date of Passing.
How would you like to receive the proclamation/citation? Requests for 6 or more proclamations/citations cannot be mailed and must be picked up at the Arundel Center*
In submitting this request, I understand that 21 days notice is required for all proclamation/citation requests. Due to formatting and space limitations, the County Executive's Office reserves the right to edit wording if necessary.
