The Anne Arundel County Community Care Partnership (CCP) is a service delivery system which coordinates services for the aging and disabled utilizing public and private agencies.  The purpose of the Community Care Partnership is to help frail and disabled individuals to remain sagely in the community for as long as possible.  It is the umbrella which the following programs fall under.


Services & Programs

Programs under CCP:

  • Community Personal Assistance Services (CPAS)
  • Community First Choice (CFC)
  • Home and Community Based Options Waiver (HCBOW)
  • In Home Aide Services (IHAS)
  • Living Well with Chronic Disease and Diabetes Program
  • Senior Center (SC)
  • Senior Center Plus (SC+)

Services provided by programs under CCP:

  • Long Term Care Case Management
  • Payment for some services depending on eligibility
  • Spiritual Care
  • Bereavement
  • Healing Touch
  • Assessment of the client
  • Development of individualized care plans
  • Arrangement for and coordination of services
  • Evaluation of the effectiveness of the services
  • Ongoing monitoring of the services
  • Empowerment of the client and/or their family
  • Continued long term management of the client's case
