  • How to Make Your Monthly Payment  PDF
  • What Forms Do I Need to Complete  PDF
  • Acknowledgement of Receipt of Parent's Manual PDF
  • Permission to Hold Discussions with School Personnel  PDF
  • Health Inventory Part I and Part II  PDF
  • Immunization Record  PDF
  • Parent’s Request to Administer Medication at School  PDF and Addendum  PDF (if your child is taking any type of medication, including over the counter)
  • Permission to Apply Over-the-Counter Creams and Ointments  PDF
  • Emergency Card  PDF
  • Receipt from "A Parent's Guide to Regulated Child Care PDF
  • “All About My Child” form  PDF
  • Health Inventory Addendum (for children under 6 years of age)  PDF
